Te Runanga o Ati Awa Ki Whakarongotai (Capital and Coast)


The winner in the Awards category Whānau Rahi was Te Runanga o Ati Awa Ki Whakarongotai for their 'Hora Te Pai Health Services'. Their whānau ora model is based on Te Whare Tapa Whā and the creation of whānau ora health plans with the client at the centre. The plan, the goals, the timeframes and the involvement of appropriate whānau members is agreed on and signed off by the client All plans are held in a central file, accessible by all whanau ora nurses and clients are encouraged to use their plans with other health professionals where appropriate.

This whānau ora model underpins the vision of Hora Te Pai Health Services to see Māori whanau enjoy equality in health and taking charge of their own health.

After much research, consultation and IT evaluation, Hora Te Pai Health Services, which is still in its early stages, is an impressive whanau ora model that sets plans for their clients and an exceptionally challenging and exciting plan for them as well.
